Discipleship Counseling Introductory Questionnaire

Getting to Know YOU! 

 A. Would you please define Eternal Life and provide Scripture references if you are able?

2. Would you share with me your life history and the messages these life events and people have caused you to think, choose, and feel – about yourself, others, and God?

A. Who are you and how does this make you feel?

B. Who is God?  What do you like or dislike about God?  What do you like or dislike about yourself?

C. Would you please list every person who has influenced your life both positively and negatively?  How have and do these people cause you to feel?  {Yes, this is to include every person since birth, as far back as you can remember.}

D. List every event within your life that has influenced how you think, choose, and/or feel {as far back as you can remember}.

E. Let's dialog about finance.  How much money do you have on you today?  How much money do you make?  How do you support yourself and yours?  How do you spend this money (provide details)?  {This question is not to be nosey; yet your answer will provide hints into your functional beliefs.

3. Would you explain in detail what you believe about sex and how you feel about sex?

Include the following information in your answer about how you think, choose, and feel:

a.   The physical aspects of sex…  

b.   The emotional aspects of sex…

c.   The spiritual aspects of sex…

d.   Do you believe or feel that masturbation is an acceptable activity and expression of your sexuality or within the boundaries of a sexual relationship; why or why not?

e.   Do you believe or feel that God is a part of the marital sexual relationship?  If so, how?

4. Would you read and explain in writing your understanding of the letter entitled Ephesians contained within the New Testament of the Bible?

A. Spiritual Truths – Chapters 1-3

B. What do these truths look like in real day to day life? – Chapters 4 -6

C.  How many times in Ephesians is the phrase, “In Christ” used or referenced?  Take 3 of these references and share what these truths mean to you?

5. Would you please read and share your thoughts on Galatians 2:20 and Isaiah 40:31 and Colossians 1:27?

6. Are you willing to accept the Bible as God’s love letter written to you with His instructions and guidance for all of life; even when you do not like what it requires?

A. Are you willing to actively accept the Word of God as THE complete standard for your life; specifically in ALL of your relationships (both intimate and causal)?

B. Do you believe God loves you?  Please share in writing what you think, choose, and feel about God’s love for you?

C. What does grace look like in your life – that is in the people, events, and functional beliefs of your life?

7. Are you desperate to experience God and His Truth as THE only solution for the situation you find yourself in?  Explain…
